Mighty Oaks Therapies is IN-NETWORK with Blue Cross Blue Shield and Cigna for pediatric occupational therapy services and accepts the following insurance plans:

• Blue Essentials HMO
• Blue Advantage HMO
• Cigna PPO

Mighty Oaks Therapies is out-of-network for all other insurances, including United Healthcare, Humana, Tricare, and all Texas Medicaid plans.

If your private insurance policy through Aetna or UHC has a provision to cover services with out-of-network OT, benefits may be payable to you once your out-of-network deductible is met. Therapy services through Mighty Oaks Therapies will be at the private pay rate and payment will be due by the date that services were rendered. Mighty Oaks Therapies can provide you with a PAID invoice monthly for the purposes of filing on your own policy for reimbursement purposes.
