To become a client for homeopathic services, please download to your computer either the three Adult forms (for 18+ clients) or three Child forms (for ages 0-17) and complete them prior to you/your child’s appointment. These are fillable pdfs you can type right into them and save them once they’ve been completed.

Two options to return your completed and signed documents:

1) Your forms can be emailed back to Mighty Oaks Therapies using the email hp@mightyoakstherapies.com
2) You can drag-and-drop your completed forms to a Contact Us inquiry on the left hand side of the “Contact Us” page of this website.
